2. Staff Personnel Records including the current photo, approved in the workplace - 2 copies

3. The extract of the Protocol on approval of Dissertation Subject at the Republican Council of Organization and Coordination of Scientific Researches - 2 copies

4. The extract of the Protocol on approval of Dissertation Subject and science supervisor (consultant) - 2 copies

5. Card on giving the doctoral exams (for Ph.D. applicants) - 2 copies

6.  The required number of dissertation’s bound copies and manuscript abstract (title pages of all copies of dissertation and cover page of synopsis should be signed by the applicant)

7. Approved copies (2) of all relevant diplomas, certificates, and transcripts for the degree Doctor of Philosophy applicants (In addition to, certificates "equivalent" issued by the Ministry of  Education of Azerbaijan Republicfor persons, who studied in abroad, including those who studied in the CIS countries)

Approved copies (2) of the diploma of PhD degree for applicants, who claimed to be Doctor of Science (In addition to, certificates "equivalent" issued by the Commission for persons, who received the scientific degree in abroad, including those who studied in the CIS countries)

8. The reference of the institution, where the applicant has been attached and dissertation work implemented (2 copies), reviewers written reviews (a copy each of them), compact discs recorded in the course of the video footage of the discussion- 2 copies

Review is drawn in the form of the extract of chair meeting’s (laboratory, department, council) protocol, approved by the head of the institution and stamped. 

Protocol is signed by the chairman and secretary of the meeting, their signatures are certified by the Scientific Secretary of the institution.

9. Testimonial on scientific-pedagogical and production activities of the applicant - 2 copies, the reference is given to the main workplace, signed by the head of the institution (enterprise), approved official seal and dated.

10. Reference of scientific supervisor (consultant) - 2 copies 

11. List of Scientific publications published on dissertation subject - 2 copies 

12. The articles, conference materials and their electronic copies (including content), monographs and books published on the subject of the dissertation are presented in a separate folder - 2 copies

The original of scientific publications, published in abroad should be submitted. In the absence of the original of scientific publications, written information, reflecting the exact address of the website and the internet address should be attach to the original of the scientific publication and this address of the website should be indicated in the list of scientific works.
